d522d135a1 Removed PHP 3 information from the manual. Goodbye PHP 3, thanks for all the memories! 
This commit covers many files.
The CVS file phpweb/manual/php3.php will exist to store [most of] this info.

- Moved from reference.xml to book.xml:
   - The intro text (partintro)
 - Moved from reference.xml to setup.xml:
   - The requirements, installation, and other setup entities
 - Changed constants.xml and examples.xml to be appendices
 - Changed the intro ID from <extname>.intro to intro.<extname>

as UNIX is a registered trademark by the Open Group. References to UNIX

307d38d980 * Made clear this sets error_reporting directive at runtime (like ini_set) 
* Removed bitmask example section, constants are preferred.
  Bitmask use is still explained.
