Hannes Magnusson b3976c1251 MFB: Upgrade to the new-reference-structure
- Moved the intro from reference.xml to book.xml
    - Change the intro ID to intro.<extname>
    - Moved examples into its own chapter
		Note: Renamed the example ID to dbm.examples-basic
    - Moved the setup sections to setup.xml
    - Added empty constants <appendix>

These functions allow you to store records stored in a dbm-style
database. This type of database (supported by the Berkeley DB,
<link xlink:href="&url.gdbm;">GDBM</link>, and some system libraries,
as well as a built-in flatfile library) stores key/value pairs
(as opposed to the full-blown records supported by relational
databases).
