- Moved the intro from reference.xml to book.xml
- Change the intro ID to intro.<extname>
- Moved the requirements section to setup.xml
- Moved the configure entity (installation section) to setup.xml
- Added empty configuration/resources section to setup.xml
- Added empty constants <appendix>
