From now on, manual.ced will need to be found at ~/.phpdoc/manual.ced.

- (Created missing setup sections in setup.xml, if any)
- Moved the intro to book.xml
- Changed the intro ID from <extname>.intro to intro.<extname>
- Moved hw.apache to its own chapter (apache.xml)
- Moved the constants entity to book.xml
- Changed constants.xml to be an appendix
- Moved the setup stuff to setup.xml
NOTE: That weirdo "todo" section in reference.xml is still there
