eda3b067eb no more "FIXME PHP_6" todo comments

af4410a7e1 Normalized the sgml-default-dtd-file local-variable line for those 
still using this, after discussion on the phpdoc list.
From now on, manual.ced will need to be found at ~/.phpdoc/manual.ced.

67b72af27e Noted that the maxlen param is actually not -1 but is instead an 
internal value (PHP_STREAM_COPY_ALL) and may not be directly specified
from within a script.
Addresses bug  (although perhaps this is better fixed in the long 
term by making file_get_contents() behave normally).
