6dfe076725 xi:include INI settings documented elsewhere in ini.list 
We do not xi:include core INI settings, since we want to keep
alphabetic sort order (well, roughly at least).

We do add xi:fallback elements to all xi:includes, just in case.

af4410a7e1 Normalized the sgml-default-dtd-file local-variable line for those 
still using this, after discussion on the phpdoc list.
From now on, manual.ced will need to be found at ~/.phpdoc/manual.ced.

833e9936ee - Upgrade PDO to the new-oo-style 
- Change IDs from function.Cass-MethoName to class.methodname
  - Change <xref>s to PDO methods to use <function>s
  - Change methodsynopsis to the new style
  - Change the ctor to use <constructorsynopsis>
  - Remove ref.pdo (old ref.pdo linkends point to intro.pdo now)

52accafdfe major update of INI stuff: 
with the update in the software, we are now able to catch more ini entries that were using the magic cfg_get_*()
minor tune in script

we are still lacking a better PECL integration, which is in my TODO list

0f3b624569 Begin splitting PDO driver docs into separate manual sections. 
Implement sorted Function Reference section of manual table of contents;
  this may need more work for non-English manuals.
Fix mysqli title so it sorts correctly in sorted table of contents.
