From now on, manual.ced will need to be found at ~/.phpdoc/manual.ced.

- (Created missing setup sections in setup.xml, if any)
- Moved the intro to book.xml
- Changed the intro ID from <extname>.intro to intro.<extname>
- Moved the constants entity to book.xml
- Changed constants.xml to be an appendix
- Moved newt.examles to its own chapter (examples.xml)
NOTE: The chapter uses the newt.examples Id,
the orginal newt.examples section is now called newt.examples-usage
- Moved the requirements & resources sections to setup.xml
- Moved the configure entity setup.xml
