5a3f0c18ac Add basic parameter entries to core DTrace probes. 
Add bulk of OCI8 DTrace section.
    Add OCI8 2.0 oci_get_implicit_resultset and examples.
    Remove OCI8 back references to pre-5.0 aliases.
    Update OCI8 LIMIT-equivalent example with new Oracle 12c syntax.
    Move OCI8 FAN to separate section.
    Remove some OCI8 <emphasis> again, since it incorrectly bolds part of the product names.

cd110607fd Updates for OCI8 2.0, Oracle 12c (including mentioning the Oracle oraaccess.xml configuration & auto-tuning file), and the Windows OCI8 2.0 DLLs. Also various small clarifications from my todo list.

af4410a7e1 Normalized the sgml-default-dtd-file local-variable line for those 
still using this, after discussion on the phpdoc list.
From now on, manual.ced will need to be found at ~/.phpdoc/manual.ced.

0912b4f4c5 MFB: Upgrade to the new-reference-structure 
- Split reference.xml into book.xml, setup.xml, connection.xml, datatypes.xml, examples.xml
- Moved from reference.xml:
  - To book.xml: The intro text (partintro), and link to various pages
  - To setup.xml: requirements, configuration, and added installation and resources
  - To connection.xml: The connection handling section to a new chapter
  - To datatypes.xml: The data type section to a new chapter
  - To examples.xml: The examples section to a new chapter
- Changed the constants section to be an <appendix>
- Changed the intro ID from <extname>.intro to intro.<extname>
