Hint at missing ext DLL dependency DLLs in troubleshooting section 
This is likely one of the biggest issues: users put the extension DLL
into `extension_dir`, but forget to put dependency DLLs in the path.

Remove old install docs (#662) 
* Remove docs for Apache 1
* Probably Apache2 doc fix
* HPUX is effectively dead, let's remove the docs
* Remove outdated info for Solaris/OpenBSD

We probably should improve those and/or drop them completely

* FPM
* Remove rackspace, it is not linked anywhere
* Update PECL
* INI configuration

af4410a7e1 Normalized the sgml-default-dtd-file local-variable line for those 
still using this, after discussion on the phpdoc list.
From now on, manual.ced will need to be found at ~/.phpdoc/manual.ced.

2a314ad163 Removed references to pecl4win as it died. And updated information with 
the current status, which is: We do not offer Windows binaries for PECL
extensions. In the future pecl.php.net will offer such downloads.

2d80dab5c4 Use the pecl command instead of the pear command. 
Feel free to research the history. It appears 1.4.0 introduced pecl, and
made pear not install PECL extensions. They both use pearcmd.php currently.

The pear command gives a warning for PECL extensions so users shouldn't
be too confused without version information.
