From now on, manual.ced will need to be found at ~/.phpdoc/manual.ced.

- Change IDs from function.Cass-MethoName to class.methodname
- Change <xref>s to PDO methods to use <function>s
- Change methodsynopsis to the new style
- Change the ctor to use <constructorsynopsis>
- Remove ref.pdo (old ref.pdo linkends point to intro.pdo now)
