- Moved the intro to book.xml
- Changed the intro ID from <extname>.intro to intro.<extname>

Moved wrapper descriptions into the wrapper appendix.
I plan to add more details, such as the the file_get_wrapper_data()
structures for each wrapper, so this is a positive move in that direction.
