966013e33d Integrate user note 48364, since it's correct about the limitations of

cf1826d271 Fix doc bug #62950 (PHP documentation broken link), and change the displayed 
link text from the URIs to something more descriptive, since the new URIs are
actually uglier than the old ones (thanks, Microsoft).

d0223af67d Fix doc bug #62414 (Out of date docs for CRYPT_BLOWFISH). I'd greatly 
appreciate a review on this, since I'm not sure it's as well worded or laid out
as it could be -- the one paragraph per hash convention is starting to get a
little tight.

a9b26c34e4 Display language constructs without parentheses 
Side effect of this change is that include, require and return are now properly linked. They weren't because they don't live inside <refentry> so PhD doesn't know about them.

4effeb4d1f HTML functions use encoding, not character set 
E.g. ISO-8859-1 is both character set and encoding but UTF-8 is encoding of Unicode character set.

71b49c3573 - html_entity_decode claimed to have had its default encoding changed from latin1 to UTF-8 in 
PHP 5.3.3; this is not true, as the change was only operated in PHP 5.4. Changed accordingly.
- Documented the behavior of encoding autodection when an empty string is passed as the
  "charset" parameter.

00b427c07b Fix doc bug #53678 (Formatting Suggestion: "count" parameter). Also fix up the 
str_ireplace() manual page, which seems to have lost paragraphs here and there
for some reason, by copying verbiage across from the str_replace() page and
changing it as required, and fix a grammatical error in the paragraph about
using arrays as search and replacement values.

a3e8136e4e 1) Fix the current Blowfish documentation. It was pretty horribly wrong, which Solar Designer pointed out to us quite a while back [1]. I more or less copied the way the Perl package Crypt::Eksblowfish::Bcrypt documents it, with a few modifications to make it fit better with how PHP uses it. 
2) Document the new (as of 5.3.2) CRYPT_SHA_256 and CRYPT_SHA_512 constants (and the salt format that goes with them), as well as include the examples.

768a81cfaa There was a lot of confusion in the user notes about the !== operator. An 
example was added using the !== operator in hopes of clearing some of it up.

Each example now has its own example box and title as well.
